You have disregarded the template provided when you create a topic in this category, doing so is ill advised because we will not process your appeal. Look at this example appeal: What ban appeals could be, the bold questions, the empty line after the answers to each, these are required. If you want to get the format again, go the ban appeals section, click create topic and copy it, but don’t actually create a topic, then edit this ban appeal with that format.
