How often it happens
Any time that a player reselects a class in Zombies on Round 1.
Any screenshots or videos showing the bug
Demonstration: I have the Wall of Flesh mask vanity item on DTP, which should be active in Zombies lobbies.
However, if you reselect a class during any game, it disappears. (the retextured WoF mask in that clip shown)
In a similar vein, if you’re respawned without being revived, you don’t have your vanity item(s).
