Old One's Army not starting despite the eternia crystal activating the event start

How often it happens
This is the first time I have encountered the problem. However, due to the fact that it can happen, I’d imagine that this can be replicated in some way somehow.

It appears that the eternia crystal was used to start this event, but due to some unknown error it is perpetually stuck in being activated, though the event does not occur. This is the first time I have checked the Old One’s Army arena and found this issue, so I do not know if it has happened previously, though I’d imagine it has. I also do not know if there is already some measure in place to remedy the issue, such as some command being routinely executed to end the event after a certain length of time, but if not this may be something we need to look into. I think max NPC spawns causes that, I would have to use clear command to it

Yep using /clear fixes the OOA issue. There’s another issue that was found though. There is this singular spiked slime that exists at the OOA arena. You can teleport to it, and /tpnpc “” acknowledges that it exists, but using /clear does not work on it. It shows this slime taking up the 200th npc slot, I am unsure if this is the source of the OOA issue or what. Here is a video showing the spiked slime.

Here is what I think happened:
At some point, a bunch of Destroyers or Planteras were spawned, since they create multiple The Destroyer, Plantera's Hook, and Plantera's Tentacle entitles. This filled up the NPC slot capacity to 198 (or some number near the NPC limit), causing the OOA event to bug-out since spawning it creates 3 different NPCs.

By the way, there is a Stardust Pillar in the server that doesn’t appear to spawn any Stardust NPCs. It just exists and fills up NPC slot 2.
