The entire ban appeal format is missing from your appeal; to get it, click New Topic under the Ban Appeals category, copy the entire thing, and then when editing your existing appeal, paste it in over what you currently have, then fill it out to the best of your abilities.
Do not post a new appeal; edit your existing one. Post a reply here once you have made the corrections.
